At Calvary Church, we love sharing the stories of how God is moving in and through our church family. Photos and videos help us celebrate life change, community, worship, and ministry moments that encourage others and point people to what God is doing here. This Media Usage Waiver simply helps us know your preferences regarding the use of photos or video that may include you or members of your household. We’d love for you to take a moment to complete the form either way, whether you choose to give permission or not, so we can honor your wishes well and care for your information responsibly.

Agreement 

We/I hereby grant Calvary Church and its elder board, governing body, employees, representatives, independent contractors, and/or assigns (hereinafter the “Church”), permission to photograph or film my/our image, likeness, or depictions, and/or that of my/our minor child or children, if applicable (hereinafter the “Media”).

We/I hereby grant permission to the Church to edit, crop, or retouch such Media, and waive any right to inspect the Media prior to its use by the Church. 

We/I hereby consent to and permit the Media of us/me and our/my minor children to be used by the Church, for any purpose, including promotional purposes, and in any medium, including print and electronic.

We/I understand and consent to the Church using such Media with or without associating names with the Media.

We/I hereby discharge and release the Church from any claim for damages of any kind, including but not limited to, invasion of privacy, defamation, false light or misappropriation of name, likeness, or image, arising out of the use or publication of the Media of me/us or those of my/our minor children by the Church. All grants of permission and consents, and all covenants, agreements and understandings contained herein are revocable upon written notice to the
Church.

We/I acknowledge and represent that we/I are over the age of 18, have read this Media Usage Waiver and Release, that we/I understand its provisions, and that we/I have signed it knowingly and voluntarily on my behalf and/or on behalf of my/our minor children, if applicable.
